The Administrative Document Preparation Package includes preparation of the following four documents based solely on the information provided by the purchaser:

• Last Will and Testament
• Living Will
• Medical Power of Attorney / Health Care Power of Attorney
• Durable Power of Attorney

The package also includes general instructions regarding signing, witnessing, notarization, and execution.

Document-preparation services are administrative in nature. Chantie Marie does not determine which documents, provisions, beneficiaries, fiduciaries, powers, or legal strategies are appropriate for any purchaser and does not interpret the legal effect of any document.

Laws governing wills, powers of attorney, health-care directives, witnessing, notarization, execution, and estate-planning documents vary by state and may change over time.
